    Read patient symptom logs and wellness check-ins

    Every care plan asks the patient a single question once a day: "How are you feeling?" Five-point scale, Rough to Great. If they tap 1, 2, or 3 the surface reveals a specific-symptom form so they can log what's actually wrong. If they tap 4 or 5 we log the wellness score alone and thank them. The whole thing takes the patient seven seconds.

    On your side, the Patients tab has a "Needs attention" panel that surfaces the things worth looking at. We do not page you on every log; we surface patterns. Calling a patient is a clinical judgment call, not an automated trigger.

    The four sections

    Needs attention. Patients who logged severity 4 or 5 on any specific symptom in the last 48 hours. The recent timestamp is the trigger; older high-severity logs roll off automatically. Click the row to see the full log history for that patient.

    Worsening trend. Patients whose symptom severity has climbed for three or more consecutive days after day 3 of recovery. This catches the "started fine, getting worse" pattern that's the most clinically useful signal, whereas day-1 spikes are usually expected post-op pain.

    Silent patients. Patients who were logging regularly then went quiet mid-recovery. Silence isn't always bad (they may just feel fine and forgot), but combined with an earlier worsening trend it's worth a call.

    Benchmarks. Per-procedure averages so you can tell whether a given patient is tracking better or worse than the typical outcome for that procedure across your roster. Useful for setting expectations with patients who worry they're behind.

    Anonymous wellness notes

    Patients can mark any note as anonymous in clinic reporting views. When they do, you see "Anonymous patient" with an ANONYMOUS pill instead of their name. The note text, score, procedure type, and day of recovery are all still visible, so you have everything you need to spot a pattern. You just can't click through to the patient identity from the panel.

    We added this in May 2026 because some patients want to share real recovery feedback (especially mood, fatigue, sleep) without it being attributed to them in a clinical record. The audit log still records the underlying care plan ID for HIPAA trail purposes; the dashboard UI just doesn't surface it.

    The five wellness levels

    Patients pick one of these every day:

    • 1 - Rough. Worse than expected.
    • 2 - Struggling. Not great.
    • 3 - Neutral. About what they expected.
    • 4 - Good. Better than expected.
    • 5 - Great. Strong recovery.

    What to do when something concerns you

    Call the patient. Their phone number is on the patient detail panel. The QR Rx surface is intentionally one-way: there is no in-app messaging back to the clinic and no escalation button on the patient side. That's a deliberate clinical and legal choice: real concerns get a phone call from a real person, not a chat thread.

    Document the call in your own clinic notes outside QR Rx. The symptom log is a patient self-report; the clinical chart lives in your EHR.
