A care plan is the digital recovery sheet you'd otherwise print and hand to a patient. Same content, better format: it's branded with your logo and colors, it answers patient questions without phone calls, and it prompts the patient to leave a review at the moment they're most likely to. The first one takes a minute to send. Every plan after that takes about thirty seconds.
You'll need three things: the patient's first and last name, their date of birth, and either their email or phone number. DOB is the unlock secret patients type on their phone after scanning the QR code, so it has to be exactly what the patient will enter. Email or phone is how we deliver the plan; either works, both is best.
The email and SMS each carry the same three things: a link to their care plan, a QR code (works for the printed discharge sheet too), and a 6-digit access PIN. When they tap the link or scan the QR, they type the PIN and their date of birth to unlock. No app to install, no account to create, no password.
Click Review instead of Send on the confirm step. You can tweak any instruction, add a milestone, edit a medication directive, or write a custom note before the plan goes out. Edits don't change the underlying template, so the next plan from the same template still starts from the original copy. To make a permanent change, edit the template from Library → Templates.
Can I send the same plan to multiple patients at once? Yes. Use Bulk Create from the New Care Plan menu and paste a tab- or comma-delimited list. Or import a CSV from your PMS export.
What if I made a typo in the patient's name? Open the patient row in Patients, click Edit, fix the field, save. The patient's existing plan picks up the change on next scan.
What if the patient never opens the plan? We send a nudge SMS at 24 and 72 hours of no-open. After that the plan stays in your roster but goes quiet. You can resend manually any time.
